If you have questions regarding your

Prescription Refills:
If you have a prescription that needs to be refilled, call your pharmacy and give the pharmacist the following information:

  • Your Name
  • Prescription Number
  • Name of the Medication

If necessary, your pharmacist will call our office for verification. It is important for our office to be up-to-date on any medication changes made by other physicians who may also be treating you. Please advise us of any changes and always bring your medications with you on each office visit.

Medical Records:
There may be a charge for medical records to be released and a release form may need to be signed. Contact medical records at 972-720-5830.
